2025-26 Physical Information

Open to ALL current and incoming SJJ students. All students are encouraged to take advantage of this time to complete their physicals required for the upcoming school year! 

For any incoming students, please let us know if you have a time constraint and we can facilitate accordingly. 

When: Wednesday, May 21

Time: 11 a.m. - 1 p.m.

Where: St. John's Jesuit Auxiliary Gym/Trainer's Room

Cost: $25 cash only 

Please print a bring a copy of pages 4 and 5 of the Preparticipation Physical Evaluation form. The form can be found here.

Once physicals are completed, parents are responsible for uploading the physical to FinalForms. No paper copies will be accepted or turned in at the physical. Please contact Athletic Trainer Ashley Wagner if any assistance is needed uploading the physical.
